Overview
When no existing form fits, you can build a completely custom form from scratch. You name the form, set its visibility and access restrictions, then add fields and tables of the exact types you need—such as product selection, photo, or text—and mark fields as required. Building from scratch gives you full control over what data is collected and how.
Before You Start
You need access to the Forms area and permission to create forms. Plan the fields and tables you need and how access should be restricted before you start.
How to Create a Form from Scratch
Configure Custom Forms in FieldPie Application
Go to app.fieldpie.com
- To create one, Click “Forms” to open the forms module section in FieldPie.
- From the top-right corner of the page, click the Forms icon to access the Form Edit menu or you can tap on profile icon
- And select Customize form to enter Form edit menu.
- You can see all the forms that have been created before and edit them. To add a new form click on the plus icon.
- You can select from one of the templates or you can create a form from scratch.
- Start with entering the form name from the top left field.
- Click “Visibility” to configure who can see and use this form.

- You can restrict this form for “main customers
- You can choose the frequency of this form and customize it.
- Click “+ Add Table” to insert a new table into your custom form. With this data entry type, the fields you add to the table will be displayed as column headers, allowing users to enter data in a structured table format.
- Enter your table name. This name will appear as the sheet name when you export the form data as a report.
- Enter your table column headers.
- Since we are creating a table to collect product information, enter “Product Name” as the name of the first field in the table.
- Click “Product Selection” to configure product-related options in the form.
- Click “+ Add Field To Table” to insert a new field into the current table.
- Click here to proceed with adding the new field to the table.
- Add another column header to your table.
- Click “Integer” to set the field type as an integer for numerical input.

- If you Click “Add rows for each option of first table field when form is created” it will enable automatic row addition based on options.
- To add another entry area on the forms outside of the table, click “add field”
- Select “Photo” as the field type to allow users to upload or capture a photo.
- Click “Save” to finalize and save all your custom form settings.

- Choose the field type that matches the data—Product Selection, Photo, Text, and more are available.
- Mark fields Required when they must be completed before submitting.
- Use tables to group repeating sets of fields, like a photo list.
- Plan the structure before building so the form flows logically.
- You can use the template forms and edit them, or you can Click “CREATE FROM Scratch” to create a new one
- If needed, you can restrict form access based on users or stores. This allows you to create forms for specific roles or skills, or make them available only for selected retailers, stores, states, or other store groups.
- To collect data through the Form module, you first need to define the information users are required to provide. This information can be collected through individual fields or organized in a table format, depending on your requirements.
